Cassandra Jones spent over a decade at the bedside as a Progressive Care Certified Nurse, caring for medically complex patients and their families during some of the most vulnerable moments of their lives. Time and again, she watched families leave the hospital overwhelmed — unsure of next steps, unfamiliar with care options, and without a trusted guide to help them navigate the road ahead.
That experience became the foundation of Extended Living Solutions. Cassandra launched the practice with a clear mission: to bring clinical knowledge, compassionate advocacy, and personalized guidance to every family facing the challenges of aging and long-term care.
Today, Cassandra and her team serve families across the region, providing the kind of expert, unhurried support that makes a real difference — from hospital discharge planning to assisted living placement, care coordination, and beyond.
